UMYF Permission Form (2009-2010) - needs to be on file for youth in order to participate in UMYF activities. Includes medical permissions and parent signatures.
Youth Covenants (2009-2010) - needs to be on file for youth in order to participate in UMYF activities. Includes the expectations for youth in UMYF.
ReCreation Commitment (2010) - to reserve your space for the annual youth mission trip.
Required ReCreation forms (2010) - paperwork to be filled out and notarized for ReCreation participants.
Photo Opt-Out - an optional form if parents do not want pictures of their youth to appear on the website.
